Housing Instructions

The official company for the 2005 California State PTA Convention housing is

No FAXed or E-mailed housing forms will be accepted before 8:00 AM,
December 16, 2004. Any FAXed or E-mailed forms received before
that time and date by SCVB Housing will be returned.
Forms received through the mail will be held and opened at the end of the day.

All reservations and cancellations must be made through SCVB Housing.

HOTEL ROOM RESERVATIONS

One Housing Reservation form must be completed for each room requested. You may copy the form if necessary.
Form must list delegate’s full name; if a delegate is unknown, a name must be submitted (e.g., out-going president, 1st VP, etc.). Names may be changed at a later date.
When sharing a room, add additional names on lines provided. Do not use another housing form. There is a maximum of four (4) people per room. Confirmations will be sent to the first person listed within 48 hours.
Complete your hotel choices by placing the number 1-7 in your order of preference beside the hotels listed.
Aguarantee must accompany each reservation, either a personal credit card number or a deposit check in the amount of $150.00 made payable to SCVB Housing. You must supply a personal credit card number if you are making a reservation by FAX, E-mail or on the Web. Your personal credit card is only a form of guarantee. You may pay the hotel with a unit, council or district PTA check upon arrival.
When several delegates wish to be housed at the same hotel, you will need to mail, E-mail or FAX all the completed housing forms with guarantees (one for each room) along with a cover letter stating that you wish to be placed in the same hotel. The housing company will do everything possible to honor your request.
Make a copy of the form and retain for your records.
FAX or E-mail completed housing forms after 8:00 AM on December 16, 2004. Forms received through the mail before December 16, 2004 will be held and opened after the mail, FAX, online and E-mails are received on the first day.
No telephone reservations will be accepted.
Forms must be submitted to SCVB Housing no later than April 4, 2005.

CANCELLATIONS

All changes and cancellations must be made through SCVB Housing until April 4, 2005. After April 4, 2005, you must call the hotel directly. All changes should be in writing to SCVB Housing, via E-mail jreilly@cityofsacramento.org or FAX 916.808.8414.
For any cancellations made on or after February 28, 2005 (60 days before your arrival date), your deposit will be refunded less a $50.00 processing fee. If guaranteed by a credit card your card will be charged a $50.00 processing fee. Cancellations made after 72 hours prior to arrival date will be charged one night’s room and tax and will be billed to your credit card or deducted from your deposit.
