• Included tally form completed by unit convention chairman or PTA president?
• Submitted all registration material and payment to the next level PTA by council/district
• deadline or followed Advance Registration procedure if after council/district deadline?
• Included the Parent’s Approval and Consent to Treatment form for student delegates?
• Ordered Starry, Starry Night Ticket?
• Submitted Official Housing Registration Form by deadline to Housing Company (DO NOT
• send this form to the California State PTA)?
