Registration Procedures

Complete and forward form and payment to the convention chairman or PTA president.

EARLY REGISTRATION

  • Form and payment are due by established council and district PTA deadline dates. Verify date with council and/or district PTA.
  • Use one Registration Form per person. Make copies of form as needed.
  • If delegate is unknown, please enter “School Name A” in the name blank for the person’s first name and Delegate Aas last name; then continue with “School Name B, C,” etc., for subsequent names on additional forms if more than one delegate is unknown. (Example: “Meadows A Delegate A” for one delegate from Meadows Elementary). Please shorten school name to less than 10 letters
  • Attendees under 18 years of age must have a signed Parent’s Approval and Consent to Treatment form (available from your district PTA).
  • Fill in name and NATIONAL PTA identification number of either the unit or council or district PTA you are representing. If the identification number is unknown, contact district PTA or California State PTA for assistance in acquiring the number.
  • Check box to remove name and address from mailing list purchased by exhibitors. Checking this box will not remove name from lists provided when delegate has their barcoded name tag swiped in exhibit hall.
  • List the first and second convention conference choices for the days you are attending.
  • Check Special-Needs Information, if applicable.
  • Determine registration category and enter cost on appropriate line.
  • If you are registering as a one-day attendee, please specify the day you are attending.
  • If attending “Starry, Starry Night,” enter amount specified.
  • Total California State PTA Convention fees and enter grand total amount on form.

ADDITIONAL FEES FOR COUNCIL/DISTRICT PTAs

The convention chairman or PTA president will know if there are additional fees for council and/or district PTA. Contact council or district PTA for more information.

Early Registration Payment Procedure

Follow these procedures if you are registering prior to established council or district PTA deadline date. Early registration is due in the California State PTA office from district PTA by January 31, 2005.

  • Convention chairman should verify deadline date for submission of forms, payment process and deadlines with council or district PTA.
  • Determine fees for the California State PTA and enter the amount on the tally form and the bottom right corner of the Registration Form. Council or district PTA fees should be calculated on tally form in applicable columns.
  • Submit all registration materials and total payment (state, district, and council) to the convention chairman or PTA president.
  • Attach a copy of your current 2004-2005 PTA membership card to form.
  • It is the responsibility of the convention chairman or president to submit the paperwork and payment to the next level PTA as directed. Convention chairman instructions can be found on page 20.
  • DO NOT mail checks and forms directly to the California State PTA office.

Advance Registration Payment Procedure

Follow this two-step payment procedure if you are registering after your district PTA deadline date, and before April 12, 2005. Follow the instructions for Early Registration, you must choose the Advance Registration fee amount.

NOTE: If the April 12, 2005 deadline date is missed, then you must register on-site.

Submit to California State PTA to arrive no later than April 12, 2005:

  • Completed Registration Form
  • Completed Unit Tally Form
  • Copy of your current 2004-2005 PTA membership card
  • Check payable to California State PTA reflecting the Advance Registration amount and the optional “Starry, Starry Night” event. Send directly to:

Submit the following to your district PTA to arrive no later than April 12, 2005:

  • Copy of completed Registration Form and tally form.
  • Copy of your current 2004-2005 PTA membership card.
  • Payment for district/council PTA fees.

ON-SITE REGISTRATION

The California State PTA will not accept any convention registrations after the April 12, 2005 deadline. You must register onsite upon arrival and pay the on-site convention registration fees. You must show a current 2004-2005 PTA membership card or pay the non-member registration fee.

You must also pay any required council/district fees to your council/district PTA. Check with district PTA on payment procedure.
