Explanation Of Template Fields

A. LOCAL_UNIT - REQUIRED - The Unit ID Number is 8 digits. All 8 digits (including leading zeros) are required for electronic submission of local unit information (example: use 00001234, not 1234). Note: The local unit ID number will be the same for all members of that same unit.

B. NAME_PREFIX - Optional

C FIRST_NAME - REQUIRED - Maximum length is 40 characters.

D. MIDDLE_NAME - Optional. Maximum length is 40 characters.

E. LAST_NAME - REQUIRED - Maximum length is 40 characters.

F. NAME_SUFFIX - Optional

G. ADDRESS_1 - REQUIRED - Use home address whenever possible. Maximum length is 40 characters.

H. ADDRESS_2 - Optional - Use for apartment or suite number. Maximum length is 40 characters.

I. ADDRESS_3 - Optional - Maximum length is 40 characters.

J. CITY - REQUIRED - Use the correct spelling/spacing of cities (Fort Wayne, not Ft. Wayne). Maximum length is 25 characters.

K. STATE - REQUIRED - Must be two (2) characters only.

L. ZIP - REQUIRED - Use 5 digits or 9 (zip + 4) but no hyphens. Maximum length is 15 characters.

M. HOME_AREA - Recommended - Maximum length is 3 characters.

N. HOME_TELEPHONE - Recommended - Required for presidents. Use format xxxxxxx (no spaces). Maximum length is 15 characters.

O. WORK_AREA - Recommended - Maximum length is 3 characters.

P. WORK_TELEPHONE - Recommended - Use format xxxxxxx (no spaces). Maximum length is 15 characters.

Q. FAX_AREA - Optional - Maximum length is 3 characters.

R. FAX_NUMBER - Optional. Use format xxxxxxx (no spaces). Maximum length is 15 characters.

S. EMAIL is HIGHLY RECOMMENDED since this is the most efficient way for communication to take
place. Maximum length is 60 characters.

T. POSITION - REQUIRED - (LUP=Unit President, CP=Council President, DP=District President).
Abbreviations for most other positions can be provided.

U. BEGIN_DATE - REQUIRED - Format: MM/DD/YYYY.

Note: Each member's term date should begin with the date he/she joined and end with October 31 of the following year (or in the same year as the end of term, June 30).

Use the date member joined.

V. END_DATE - REQUIRED - Format: MM/DD/YYYY.
Use 10/31/200_
